Applicants must indicate the position they are applying for and provide a Personal Data Sheet (PDS) revised 2017, a Work Experience Sheet, authenticated copies of their Transcript of Records and Diploma, and a certified true copy of their authenticated Civil Service Commission (CSC) Eligibility/BAR/Board results (if applicable).

Additionally, certificates of relevant trainings/seminars attended, Service Record/Certificate of Employment, certification of units earned in postgraduate studies, performance rating in the last rating period (if applicable), and other credentials should be submitted as well.
