The folder must contain the following documents: application letter, handwritten and notarized Personal Data Sheet (CSC Form 212, Revised 2017), report of rating or board rating authenticated by PRC, transcript of records authenticated by the school registrar, original PSA copy of birth certificate, original PSA copy of marriage certificate (if applicable), and various clearances such as barangay, mayor, municipal trial court, regional trial court, police, and NBI clearance.

Applicants with deficiencies must attach a Certificate of Waiver to their application folder. Failure to comply with the documentary requirements will result in the disqualification of the applicant.

The deadline for submission of applications for Fire Officer 1 (FO1) in the Bureau of Fire Protection (BFP) in the Philippines is from April 3 to April 21, 2023.
